Notes

• Start and Stop Service Request Forms are to be filled out by property owners only. At this time, tenants are not added to accounts. 
• A one-time Set-Up Fee is charged to all new accounts per the Unified Fee Schedule.
• A Closing Fee is charged every time a request is made for a final meter reading per the Unified Fee Schedule (FMC 13.04.045).
